Rather than treating hypertension as an isolated condition defined solely by blood pressure thresholds, this approach considers the full constellation of risk factors that determine an individual's likelihood of experiencing a major cardiovascular event.

Those factors include age, sex, smoking history, cholesterol levels, blood glucose, family history, kidney function, and the presence of established cardiovascular disease. A patient whose blood pressure is well-controlled but who carries several of these additional risk factors may be classified as high-risk, and that classification can justify a more aggressive or comprehensive treatment approach.

In practical terms, this might mean your physician recommends a higher lisinopril dose to achieve a lower blood pressure target — say, below 120/80 mmHg rather than the standard 130/80 — based on your individual risk profile. Or it might mean adding a second agent to address a specific mechanism of cardiovascular risk that lisinopril alone does not fully cover.
